Job Description:
Lee Health, a premier health system in Southwest Florida, is
seeking a Home Health Clinical Manager to join our dynamic team. We
are committed to enhancing patient care by managing our home health
services effectively. The right candidate will oversee clinical
operations, ensuring that patient care is delivered in a safe,
efficient, and high-quality manner. This role requires a blend of
strong clinical expertise, leadership skills, and a deep commitment
to patient-centered care. Oversee the day-to-day clinical
operations of home health care services. Ensure compliance with all
federal, state, and local regulations and standards. Supervise and
mentor home health staff, including nurses and other healthcare
professionals. Develop and implement policies and procedures that
improve the quality of patient care. Conduct performance
evaluations and provide ongoing feedback to staff. Coordinate with
healthcare providers and administrative staff to optimize patient
outcomes. Manage patient care plans in collaboration with
multidisciplinary teams. Monitor and report on key performance
indicators related to home health care. Strong leadership and
organizational skills In-depth knowledge of clinical practices and
regulations in home health care Excellent communication and
interpersonal skills Ability to manage multiple tasks and
prioritize effectively Proficient in the use of healthcare IT
systems Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Valid Registered Nurse (RN) license in the state of
Florida Certified Home Health Manager (CHHM) preferred Basic Life
Support (BLS) Certification
