Job Description:
Encompass Health is at the forefront of integrated healthcare
services offering facility-based and home-based patient care
throughout the U.S. We are currently seeking a skilled Dietitian
who is passionate about providing tailored nutritional plans to
improve patient outcomes. In this role, you will work
collaboratively with a multidisciplinary team to assess patient
needs, develop dietary plans, and contribute to interventions that
reinforce the recovery process. Your expertise in nutrition will be
essential to the holistic health package we provide at Encompass
Health. Conduct nutritional assessments to evaluate patient needs
and establish dietary objectives. Collaborate with healthcare
professionals to integrate dietary plans with overall patient care
strategies. Monitor and document patient progress and adjust
dietary plans as needed. Educate patients and families on
nutritional principles, dietary plans, and food selection and
preparation. Attend team meetings to discuss patient care and
management. Stay updated with the latest nutritional science
research to enhance service quality. Strong understanding of
nutritional assessment tools and techniques Excellent communication
and interpersonal skills Ability to work collaboratively in a
multidisciplinary team Proficiency in creating and implementing
detailed, personalized care plans Knowledge of electronic health
record systems Medical Insurance Dental Insurance Vision Insurance
401k Registered Dietitian Nutritionist (RDN) by the Commission on
Dietetic Registration Licensed Dietitian/Nutritionist (LDN) as
required by state law Certification in specialized areas of
nutrition is a plus (e.g., Certified Nutrition Support Clinician,
Certified Diabetes Educator)
